Your name, date of birth, sex and identification. This is the one that creates your record with the clinical team — it is separate from your intake and consent form, which covers your medical history.
Registration is handled directly by our clinical system, which for safety will only run in its own window. It takes about two minutes. Close that tab when you are done and you will be back here.
